I have a copy of the *Cikitsākalikā*, edited and translated by P. V.
Sharma. It does contain your verse, and I am sending you separately a PDF
scan of the relevant pages.
It reads:
tathā coktaṃ --
"upodghātaḥ padaṃ caiva padārthaḥ padavigrahaḥ |
cālanā pratyavasthāne vyākhyā tantrasya ṣaḍvidhā ||"
tatra upodghātaḥ śāstrasamutthānasambandhābhidheyaprayojanāni |
after which it goes on to another citation from another author, probably
Vāgbhaṭa.

Literature, IIA, p.122-25. Probably fl. ca. 900-1050.
