In this particular case, 6 copies of the book are reported in various libraries in Japan.

Please also note that CiNii and the National Diet Library (link below) list the author's surname as Kendai (not Tateshiro) 健代 (born 1914):

Jonathan is of course correct that the best (and perhaps only) way to know how to read a Japanese surname is to ask the person directly (this may be difficult in this case). The book in question, however, was published privately (hence the low circulation) by a Japanese temple: Seikō-in Kiyomizu-dera 清光院清水寺. According to a website listing information on this temple (including address, phone number, etc.), the current abbot is Kendai Waō 健代和央（けんだいわおう）. This is most likely the author's son or grandson. 

Given that the National Diet Library and the temple both seem to confirm the reading of the surname, it is likely that in this case Kendai is better than Tateshiro.
