This verse is the maṅgalaśloka of the Jyotiṣa text Bṛhatpārāśarahorāśāstra, although one always has to wonder if such verses were added to manuscripts by later hands.

In a search of the GRETIL and Muktabodha digital libraries I was only able to find this variation in a tantric text kulālaśāstra

gajānanaṃ bhūtagaṇādhi sevitaṃ

kapittha jambūphalasāra bhakṣitam |

umāsutaṃ śokavināśakāraṇaṃ

namāmi vighneśvara pādapaṃkajam ||
