Below is a (rough) bibliography that my student compiled from all your suggestions long ago!  Thanks to many who responded, and to some (including Tim C. and Amy A.) who sent lengthy lists.  This is not in perfect bibliographic form, and references are listed as they were given, so I can't guarantee that everything is accurate.
